Neighborhood Health Association (NHA), a Federally Qualified Health Center (FQHC), is the largest community health center system in Northwest Ohio with partnerships that include ProMedica, Mercy Health, and University of Toledo Medical Center. Today we operate multiple health centers providing pediatrics, adult medicine, dental services, health care for the homeless, women’s health, senior care and a full service pharmacy with lab services on site.

In keeping with our mission to meet the needs of the most vulnerable in our community, we have an exciting opportunity for a manager to join our team in a new role as Community Services Manager.

General Function:

The Community Services Manager is responsible for providing the leadership of all aspects of our clinical community health outreach initiatives, including our two NHA COVID-19 testing site locations, and community based testing on-site at identified partners.

Core Responsibilities:

· Assure all services provided are in alignment with patient needs, Provider standards and NHA mission

· Oversee the daily operation of the NHA COVID-19 testing program

· Meet with leaders of our community partners to identify and implement on-site testing at community partner locations

· Oversee and schedule on-site and mobile testing teams

· Work with NHA leadership to identify ways to promote NHA COVID-19 testing initiatives to the community

· Collaboration with other departments to assure patients are connected to safety net services.

· Reporting of testing data and results to NHA Executive Management Team and county health authorities as applicable

· Review all reporting, analyze data and recommend efficiencies and changes in process where necessary

· Assuring patients receive test results in a timely manner and scheduled follow-up medical care with their Primary Care Physician

Skills and Qualifications:

· Demonstrated management/supervisory experience

· Demonstrated leadership capabilities with the ability to motivate others in a team work environment

· Must have a demonstrated track record of project management, with the ability to see projects through from concept to completion

· Must have thorough understanding of Electronic Health Record (EHR), preferably All Scripts

· Must exhibit a professional demeanor to internal and external stakeholders at all times

· Ability to meet new challenges with a positive and flexible attitude

· Strong attention to detail

· Experience in community health serving the underserved population a plus
