Community Health Worker (CHW)-Perinatal Outreach

Neighborhood Health Association (NHA), a Federally Qualified Health Center (FQHC), is the largest community health center system in Northwest Ohio with partnerships that include ProMedica, Mercy Health, and University of Toledo Medical Center. NHA has grown from a single location in 1969 to 12 medical and dental clinics throughout Lucas County. Today we operate multiple health centers including pediatrics, adult medicine, dental services, and health care for the homeless, women’s health center, senior centers and a full service pharmacy with lab services on site.

General Function

The Community Health Worker will work on a team as a Perinatal Outreach Worker with responsibilities of the care coordination of high-risk pregnant women in Lucas County.

Job Responsibilities

  • Identifies and recruits high-risk pregnant women in Lucas County to enroll in program, maintaining an active caseload of 35-40 pregnant/postpartum women according to grant guidelines
  • Completes and maintains required client records; providing accurate and timely documentation according to program guidelines
  • Implements and manages client plan based on individual needs, providing referrals to appropriate services
  • Motivates pregnant women to obtain care, connects women with systems and providers, and completes follow-up strategies to ensure that women receive needed services
  • Promotes healthy lifestyle choices by encouraging clients to manage and reduce health risk factors
  • Provides appropriate health and developmental educational information to target population
  • Completes required program documents using appropriate agency protocols
  • Performs basic clerical, computing and office duties

Miscellaneous and Special Assignments

  • Represents the Perinatal Outreach Program at community events, i.e. health fairs, etc. Serves on community committees and boards related to pregnancy and infant health as assigned by the Program Coordinator
  • Participates in-house training and training (which will require out of town and possibly overnight travel.)
  • Attends agency and program meetings when appropriate
  • Additional duties as assigned by NHA management

Qualifications

  • Certified as a Community Health Worker
  • Experience as a Community Health Worker or in social services
  • High School diploma/GED
  • Ability to read, write, comprehend and follow oral and written instruction.
  • A valid Ohio driver’s license and auto insurance with an acceptable driving record.

Must have reliable transportation and willingness to travel extensively throughout Lucas County.

  • Knowledge of basic computer functions (Microsoft Windows, Word)
  • Ability to work closely and effectively as a team player with NHA staff, community leaders and health professionals

Full-time non-exempt hourly, 37.5 hours per week $16.00/hrly, Monday through Friday no weekends or holidays
