Early Intervention Service Coordinator

Family and Child Abuse Prevention Center is seeking an Early Intervention Service Coordinator to work with babies and toddlers through age 2 with developmental delays and disabilities. A Service Coordinator is responsible for assisting families coordinate health & developmental services to children and their families while working with a multidisciplinary team of professionals. The Service Coordinator conducts family assessments as well as coordinates evaluations/assessments to determine program eligibility, develops individualized service plans and ensures the child’s and family’s needs are being met by providing additional community resources. This position requires local travel and the ability to conduct in-home services.

Applicants must meet the State of Ohio education requirement to be considered and must possess a Bachelors Degree in Early Childhood Development, Health and Human Services or Social Work.
