Housing & Economic Stabilization Case Manager

POSITION SUMMARY:

As a Housing & Economic Stabilization Case Manager, you will provide housing stability and income-focused case management services to program participants including, but not limited to: conducting assessments to identify housing and economic support services needed to maintain housing and increase economic stability; providing resources to increase income; advocating for program participants’ housing and economic needs; collaborating with the Employment Coordinator and program participants on increasing necessary skills and obtaining employment; coordinating and facilitating case conferences as needed; and making outside referrals for appropriate supportive services. The Housing & Economic Stabilization Case Manager will perform all duties and responsibilities in a manner that models the mission and vision of Beach House, Inc.

DUTIES AND RESPONSIBILITIES:

  • Provides direct services to program applicants and participants including but not limited to: processes intakes; completes housing assessments; completes annual assessments; identifies supportive services, and develops individualized housing stability plans; provides advocacy and support; demonstrates healthy landlord-tenant relationships; and educates program participants on their rights and responsibilities as a tenant
  • In collaboration with the Housing Inspection Specialist, assists with the housing search and placement process, accompanies program participants to view units, advocates for program participants with landlords while maintaining a professional and healthy landlord-tenant relationship, and assists program participants with locating valuable community and housing resources (i.e., utility assistance, furniture assistance, etc.)
  • Coordinates housing services with other service providers and makes appropriate outside referrals as needed
  • Networks with community resources for housing, employment, substance abuse treatment, mental health treatment, training, benefit assistance, etc., and makes referrals as appropriate.
  • Facilitates access to community resources and assists program participants with expanding their support networks
  • Conducts monthly case management meetings to identify goals and supportive services related to maintaining permanent housing and economic stability
  • Incorporates the program participants’ goals related to finding and maintaining permanent housing, increasing household income, and expanding support networks into an individualized housing stability plan that is reviewed monthly and updated annually
  • Maintains a working knowledge of community resources
  • Obtains signed release of information forms and communicates regularly with staff at treatment programs, substance abuse programs, and vocational programs
  • Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature
  • Maintains client and program records in accordance with applicable standards and regulations
  • Ensures a safe, professional, and supportive environment while providing case management services
  • Establishes and maintains a positive working relationship with program participants, staff, landlords, vendors, volunteers, and community members
  • Identifies and provides emergency crisis services as necessary; makes immediate assessments and responds according to accepted crisis intervention methods and techniques
  • Creates reports, gathers statistics, and submits reports per guidelines
  • Transports program participants as needed
  • Attends trainings, meetings, and community events as requested
  • This position reports to the Program Director and does not include any supervisory duties
  • Assist in the growth of LFH through active participation in fundraising events.

QUALIFICATIONS:

BSW with licensure (LSW) preferred; however, will consider a four-year degree in social service or related field plus previous experience that is directly related to the duties and responsibilities specified above.

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

  • Strong interpersonal and communication skills (both verbal and written) and the ability to work effectively with a wide range of people in a diverse community
  • Ability to monitor, assess, and record client progress and complete adjustments as necessary
  • Ability to plan, implement, and evaluate individual service plans
  • Strong knowledge of community resources or ability to become knowledgeable required
  • Strong interpersonal and organizational skills required
  • Proven ability to submit timely, detailed, and accurate case notes
  • Maintain a high level of computer skills and competency
  • Ability to gather data, compile information, and prepare reports
  • Ability to effectively manage workdays and time
  • Ability to climb stairs, lift 40-50 lbs., push/pull 50 lb. items up and downstairs.
