Housing Navigator – Case Management

Housing Navigator (Case Management):  Full -Time                           Hourly Rate:  $18.00

Purpose:  St Paul’s Community Center is seeking a high energy, compassionate individual to serve as the Housing Navigator/Case Manager for the Shelter.  This person should also have a compassion for working with individuals experiencing extreme hardship and poverty.   This individual enjoys engaging with the homeless population and the general public.  The qualified individual will perform on-going assessments, SPDAT assessments and coordinate individual service plans (ISP’s).  Duties including assisting clients to achieve personal independence in managing their basic needs and monitors progress towards identified goals.  Also, documents client’s activities through case notes, terminations, rule violations, and incident reports.  Provides assistance with linkage to formal community services.  Some typing and data entry into Homeless Management Information System (HMIS).  Other general labor duties as directed.

Minimum requirements:  BA in social work or related field.  Experience working with the homeless population.  Some experience securing public housing and other public services.  LSW preferred; Driver’s license required. Must have computer knowledge and skills, positive attitude and ability to follow directions.
