Licensed Professional Counselor (LPC)/Licensed Clinical Social Worker (LCSW)

JOB DESCRIPTION

Position Title: LPC or LCSW
Status: Contractor

The LPC or LCSW provides a mental status evaluation to determine the psychological
stability of the individual being assessed. The overall goal is to help determine whether
the person has the cognitive and psychological ability to be successfully employed.

JOB DESCRIPTION
Provides a mental status evaluation for the person being referred. Reviews information
provided by the case manager to become familiar with demographic, aptitude, and
social history information. May require providing trauma counseling services. Interviews
the client, completes the mental status evaluation and provides a written report. The
LPC/LCSW will also attend required meetings. Candidate must be a self-starter with
good computer and writing skills. Candidate must have excellent time management
skills and be able to meet deadlines.

RESPONSIBILITIES
1. Receives referral from the case manager and reviews information compiled.
2. Assess the mental and behavioral status of referred clients using Mental Health
Status Evaluation. Submit completed evaluation to Program Manager.
3. Submit written report with recommendations. Make all necessary changes to
report as required.
4. Follow-up with client as required.
5. Attends the Team Meetings.
6. Utilize company’s case management systems for documentation and reports.
7. Other duties as needed

MINIMUM EDUCATION AND EXPERIENCE QUALIFICATIONS
Master’s degree from an accredited college or university in psychology or related field or
social work and one year of experience. The Licensed Professional Counselor or
Licensed Clinical Social Worker credential is required and must be maintained.
