Licensed Social Worker

The Sight Center of Northwest Ohio has a full-time opportunity for a Social Worker to join our professional and enthusiastic team.  A 501(c)3 non-profit agency serving 18 counties in Northwest Ohio for nearly a century, our team makes a difference in hundreds of lives annually.  This position will provide both clinic and community-based services to people who are blind or visually impaired.

Qualifications: The ideal candidate will hold a bachelor’s degree in social work.  State of Ohio license (LSW) & minimum of one year of experience working with persons with adjustment to disability and/or rehabilitation issues is preferred.  Criminal (fingerprinting) and employment background investigations will be conducted as a condition of hire.

Duties:  Social Worker will provide individualized clinical treatment to people with impaired vision of all ages who reside in 18 counties of Northwest Ohio.  Social Worker is responsible for management of all client cases including development and facilitation of agency plan, funding source identification and eligibility, grant fund utilization and client outcome tracking.  Must be able to advocate for individuals who are visually impaired in employment, housing, community access, transportation and other areas as needed.  Social Worker is responsible for collection and maintenance of client data through EHR and agency internal tracking system.  Social Worker serves as an integral member of our interdisciplinary vision rehabilitation team.
