Position Title: Patient Advocate
Reports to:
  Medical Services Director
Classification: Part time, 24hrs./week – Tuesday, Wednesday and Friday, 9am-5pm

Job Summary

The advocate role at Your First Look, at Heartbeat of Toledo, is a critical piece of compassionate care, to those we serve experiencing an unplanned pregnancy. Assessing clients, gathering relevant information about their situation, providing crisis intervention, contacting and making referrals to other agencies and services are a few of the most important responsibilities of this position.

Success in this role is dependent on maintaining a compassionate and consistent demeanor while interacting with the patient, the ability to stay calm and empathize with patients when they are upset and the ability to set boundaries.  Active listening skills, asking relative questions and having strong communication skills will also be critical in this position.

Duties and Responsibilities: (this is an overview, not an exhaustive list)

  1. Meet one-on-one with patients during their intake and repeat appointments
  2. Coordinate Patient care with nurse and sonographers
  3. Follow-up with patients throughout their pregnancy
  4. Enter all patient data appropriately into patient charts into electronic system
  5. Answer phones and schedule patients
  6. Remain current on available community resources
  7. Attend staff and department meetings, as requested

Position Requirements:

  1. Degree in social work or counseling, preferred
  2. Prior experience working with people in crisis
  3. Highly organized with the ability to work fast-paced and to multi-task
  4. Understands and respects patient confidentiality
  5. Has acceptable criminal background history
