Job description
ESSENTIAL FUNCTIONS OF THE POSITION PER HUD/TLCHB GUIDELINES:

Walk in clients – screen for safety concerns, contact 2-1-1

Clients referred by 2-1-1 – if unable to meet needs, contact 2-1-1

a. Provide shelter orientation

b. Connect family (and/or individual family members) with mainstream resources

ESSENTIAL FUNCTIONS OF THE POSITION

1. Reports ANY concerns regarding child abuse or neglect to the local children’s services agency.

2. Demonstrates regular and predictable attendance

3. Monitors the entering and exiting of persons from agency and answers agency telephones.

4. Monitors resident floors by walking the halls and stairwells, hourly or as needed.

5. Performs room checks, as needed.

6. Intervenes in crises situations

7. Provides one-on-one demonstration of daily living skills to enhance client’s self-sufficiency.

8. Promotes client adherence to case plan.

9. Documents completion of shift duties, including full compliance with instructions on prior shift notes and completion of tasks.

10. Documents all interactions with clients, including but not limited to, staff and client concerns, interventions provided, and outcomes achieved.

11. Coordinates resident chore rotation and checks that chores are completed

12. Assist with meals and/or snack time.

13. Assists with curfew and resident check-in at 9pm and final break in the evening

14. Attends and contributes to staff/team meetings

15. Performs other duties as required.

KNOWLEDGE, SKILLS AND ABILITIES:

Knowledge of: Social work principles and practice, community resources, human relations, group and interpersonal dynamics and cultural diversity.

Skill in: Supporting residents in goals and tasks; demonstrating daily living skills; advocating with service providers on behalf of residents; word processing; general client assessment, documentation, and oral and written communication skills.

Ability to: Work independently with minimal supervision; bend, reach, walk short distances and walk up and down stairs; lift up to 40 lbs; prepare and maintain accurate documentation; communicate effectively; develop and maintain effective working relationships with residents and staff; use sound judgment on behalf of residents in the absence of instructions.

EQUIPMENT OPERATED:

Computer terminal; elevator; standard office equipment; standard cleaning equipment (washer, dryer, broom, mop); stove.

INHERENTLY HAZARDOUS OR PHYSICALLY DEMANDING WORKING CONDITIONS:

Potential to encounter volatile residents and/or friends/family; exposure to unclean environments; lifting risks while moving donations or resident belongings; cleaning chemicals (bleach, ammonia, etc.)
