About Sunshine:

Sunshine Communities, Inc. is a 501(c)(3) nonprofit organization located in Northwest Ohio supporting individuals with developmental disabilities through residential, vocational, clinical, therapeutic, and recreational services. These services are offered in various circumstances including at our Maumee, Ohio campus and at homes within the community. In addition to programs on campus, Sunshine individuals and staff operate Georgette’s Grounds and Gifts, a coffee shop and fair trade gift store located in downtown Maumee. Right across the road lies Sunshine Studios, an art studio space and gift store offering classes to individuals and the community.

Provides social services to individuals and families supported by Sunshine.

Qualifications and Skills for ICF-IID Social Worker:

  • Bachelor’s from an accredited School of Social Work
  • Person Centered Planning and Person-Centered thinking Training preferred
  • Previous investigation experience required
  • Previous experience with both children and adults required
  • One (1) year related work experience; two or more (2) years preferred
  • Able to lift 60 pounds
  • Valid Driver’s License, good driving record and auto insurance
  • Able to pass criminal background, drug test and physical

Essential Duties and Responsibilities include the following:

  • Facilitates admissions, transfers and discharges for residential areas. This includes reviewing applications, scheduling observations, pre-admission meetings, ensuring communication and education for staff and families, developing thorough social histories, discharge planning and discharge summaries.
  • Handles inquires and maintains waiting list for services in iCM. Updates information regularly and maintains communication with families, guardians, or agencies involved.
  • Provides referrals if Sunshine services are not appropriate.
  • Serves as an advocate for individuals
  • Provides staff training on abuse/neglect, resident rights, family dynamics, and other topics as needed
  • Maintains regular communication with nursing, clinical services, staff and families to ensure needs are met and issues addressed.
  • Participates in satisfactions surveys and follow-up
  • MUI investigations and consults. Investigates incidents per agency policies and applicable regulations, coordinates with other agencies, makes recommendations for case disposition, prevention, and follow-up
  • Provides orientation training on incident reporting, and MUI process for new staff and managers as well as ongoing training as needed
  • Monitors services provided both internally and externally through quality assurance visits to homes, vocational sites, schools
  • Maintain knowledge of current Federal and State regulations, as well as, professional standards, and make recommendations accordingly for changes to policies and procedures
  • Attend Team meetings as needed
  • Apply for benefits and ensure benefits are maintained
  • Research and provide referrals to other community agencies when specialized assistance is required
  • Represents Sunshine both internally and externally in a professional manner
